(Please Read Carefully):

  1. After reviewing the available opportunities with the student, the co-op teacher will submit the request for a co-op placement.
  2. A volunteer co-ordinator from Bloorview will contact the co-op teacher to discuss possible placement.
  3. The teacher will be provided with possible Orientation Dates for the student (usually a Wednesday or Thursday evening).
  4. The teacher or student will confirm with the co-ordinator which session the student will attend.
  5. At the end of the Orientation session, the student will:
    1. complete and submit an application form
    2. submit a “Health Care Package”.  (This can be obtained from your School Board’s Placement Facilitator)
    3. submit two professional reference letters (Please note: reference letters from previous employers or volunteer experiences are preferred over references from teachers)
    4. sign up for a 45-minute interview that will take place on another date )
  6. When the student returns for the interview they will discuss possible placements with the co-ordinator.
  7. If the student is 18 years of age or older, the student will complete Police Records Check forms.  Students will need their driver’s licence and all addresses for the last five years to complete these forms.
  8. If the student is accepted, they will complete a three-hour training session (usually a Wednesday or Thursday evening). In some cases, the student will receive placement–specific training.
  9. If accepted, the student will be given contact information for their supervisor to pass along to their teacher.
  10. The teacher will contact the supervisor directly to arrange all visits.

Note:  All offers of placement are conditional on a satisfactory police records check and reference checks.
