Bloorview Kids Rehab and the Transitions Conference Steering Committee are pleased to announce the 5th biannual conference on the subject of transitioning.

A child’s life is full of transitions through which they learn to adapt, grow and evolve.  Bloorview Kids Rehab is an international leader in transitions for children and youth with disabilities, helping guide clients and families through the everyday experiences and skills needed to become mature and confident adults.

The theme for the 2010 conference is:

Partnerships; Together for LIFE

The conference will concentrate on the following pillars of strength:

Partnerships

Performance

Participation

relationships between pediatric and adult service providers, challenges and successes

research, outcomes, evidence, evaluation, getting results

inclusion, recreation and leisure, vocation, education, medical and housing

Who is the conference for?
The conference is open to health-care professionals, community service providers, educators, researchers, caregivers, families and youth to participate.

What will be offered?
There will be a number of presentations scheduled into the conference from leaders in the field of transition. 

In addition, a Youth as Leaders forum will be offered to address the hottest issues.  More information to follow.

Where will it be held?
Bloorview Kids Rehab is hosting the conference. The sessions will be held at Bloorview and CNIB (just down the street from Bloorview).
