Clients & Families

If you have never had an appointment at Bloorview Kids Rehab, and you would like to book an appointment, your doctor must complete a medical referral form.  Our staff will review the information on the referral form and will respond to your referral request.

Physicians

Clients who have never been seen at Bloorview Kids Rehab require a medical referral.  Only completed Bloorview Kids Rehab referral forms are accepted.  If you have any questions, please call 416-424-3804 for assistance.
