Position: Intake Coordinator
Program/Service/Department: Child Development Program

Salary: Min: $21.64/hr  Target: $25.33/hr

Intake/Service Co-ordinator is a member of one of the Child Development satellite teams. The intake/service co-ordinator is the primary recipient of referrals to the team and ensures that appropriate information is obtained related to motor, communication, social, and cognitive developmental areas. The intake/service co-ordinator will act as the contact between the family and the team, until the child is seen for an assessment.  He/she will work collaboratively with the team, family, client and community services to suggest resources and ensure that the agreed upon service plan is implemented.

Key Responsibilities:

  • Receive and acknowledge receipt of request for service referrals to the Program
  • Prepare referrals for review on a weekly basis by the Program Admission group
  • Obtain additional information on referrals as indicated
  • Maintain information on the type of referrals and their status on a spreadsheet
  • Liaise with Registration Services and the team to ensure that appointments are scheduled and coordinated
  • Books appointments for accepted referrals for assessments and sends out notification of appointments to client/family and referral sources
  • Responds to referral inquiries from both internal and external service providers
  • Links clients/families to appropriate services in consultation with the admissions group and/or Program clinical staff
  • Assists in producing quarterly reports on referrals received, status of referrals and wait list for appointments
  • Other duties as required

Qualifications:

  • Minimum B.A. in health related discipline or equivalent
  • Experience with intake, service coordination and/or scheduling processes
  • Knowledge of community resources
  • Knowledge of and proficiency in use of word processing, scheduling and database software
  • Knowledge of administration practices, record management and service provision processes
  • Good organizational skills and able to set priorities
  • Able to work with all levels of management
  • Able to work independently, collaboratively and cooperatively with all clinical staff and community service providers
  • Demonstrated proficiency in the use of computer applications with an emphasis on MS Office, Outlook, and Meditech Information System Admissions module preferred
  • Proven ability to function in a busy environment with minimal supervision
